To use these forms online, you first need to have the
free Adobe Acrobat Reader, available at:

       http://www.adobe.com/products/acrobat/readstep.html

(If you have an older version of Adobe Reader, please update to the latest)

Start filling out the form by first selecting the form you want from the main Forms page.

When the form opens, move to the top of the page and read any instructions found there.

To enter information, first look at the mouse cursor.  If it looks like a small hand, then you are ready to begin.  If not, click the “Hand” button on the toolbar directly above the form.  When the cursor is correct, click your mouse on the very first field at the top of the form.  You will see a blinking line cursor.  This indicates that you can begin typing. The forms are setup to progress from one field to another in a logical fashion.

After completing one field, use the “Tab” key on your keyboard to advance to the next. Using “Shift-Tab” allows you to move backwards through the fields. Enter all the pertinent information, including the date.

Note that certain fields such as dates, phone numbers, Social Security Numbers, and Zip Codes are formatted fields.  This means that for Social Security Number, for example, there is no need to enter the dashes. Enter numbers only and the dashes will be put in for you after you exit the field.  The same holds true for phone numbers.  Zip code fields will only accept five digit zips.

The one exception to this are date fields. Dates must be entered with either slashes or dashes in the following format:

MM/DD/YYYY

Most forms provided here require signatures. Obviously, these are difficult to do when online.  Therefore, when completed, don’t forget to print the form, sign it, and mail it to the Carpenters’ Service Office.

If you have problems filling out the forms on screen, you can always print out the blank form and fill it out the old-fashioned way.
